    Note: All of the forms are Adobe Acrobat PDF files. To read and print these files, you must have a recent copy of the free and easy-to-use Acrobat Reader. Once you have a copy of Acrobat Reader, you can open the forms, fill them out on your computer, and print them, or, if you prefer, you can print them and fill them out on a typewriter. Either way, the printed and signed forms must be turned in to the School of Urban Studies and Planning office, 350 Urban Center. The forms cannot be submitted electronically.
