The Process of Becoming Eligible for Services
As required by the Americans with Disabilities Act (ADA), students who have disabilities which impact their ability to function in the classroom and want services must identify themselves to the DRC office and register for services.
In order to register with Disability Resource Center, you must:
- Make an intake/accommodation assessment appointment
- Provide appropriate evidence of your disability from a qualified medical >or psychological professional (The cost of obtaining necessary documentation is borne by the student)
Accommodations are determined on a case-by-case basis and depend on the nature of the disability and the documentation provided. If documentation is incomplete or inadequate, the office has the discretion to require additional documentation. Any cost of obtaining further documentation will be borne by the student.
Certain disabilities require additional documentation. Diagnosis forms for Attention Deficit Disorder and Functional Limitations are available at DRC.
Students with a temporary disability, which effects them in classes, must provide medical documentation on a term by term basis.
Documentation is held under strict confidence and in accordance with all laws pertaining to disability in higher education.
Learning Disability Documentation
Students with a learning disability need to present documentation from a school district or credentialed professional who tested the student no more than three years prior to current attendance.
Results must include test scores and an official write up which includes the specific diagnosis.

Off-Campus Sites
The Disability Resource Center provides coordination and consultation services for off-campus sites (i.e., the Salem Center and the Capitol Center.)
Documentation will be housed at both sites, under strict confidence and in compliance with all laws.
Student Rights and Responsibilities
The Americans with Disabilities Act requires that both students and the university have responsibility for academic accommodations.
The Disability Resource Center published Access to PSUccess Student Handbook and Faculty Handbook to assist students and faculty in understanding their rights and responsibilities in accordance with the Americans with Disabilities Act.
