Proctoring DRC Accommodated Exams

The SHAC Testing Services tests students who receive accommodations through the Disability Resource Center (DRC). The SHAC Testing Services is an option, not a requirement, for faculty and students to assist faculty with providing the required testing accommodations. Faculty may opt to provide accommodations on their own.

Proctoring Form

Please send us the completed Proctoring Form (make sure to mark the DRC box) with your exam. Thank you.

Required: Deliver or send the following materials in person or via e-mail, fax, or campus mail:

  1. The exam. The test materials will be kept secure.
  2. The Proctoring Form. Please save the form to your Desktop and, if you choose to e-mail the exam materials, send it as an attachment. This form enables us to administer the exam as you instruct for a non-DRC student on the proctoring form. The SHAC Testing Services will look up the student's accommodations and adjust the instructions accordingly (e.g. extended time). Please fill out the form completely, including the deadline, which we strictly adhere to. Students will not be permitted to test past this date without your explicit permission and will be turned away if it is past their deadline.

The SHAC Testing Services requires students registered with DRC to make appointments one week in advance.

YOUR RESPONSIBILITIES

You must do the below steps before taking an exam at the SHAC Testing Services.

1. At the beginning of each quarter, complete the following:

  • Complete the Service Request Form online. If you need help updating or entering your information, please contact the DRC at (503) 725-4150 or drc@pdx.edu. You will list your class schedule and the accommodations that DRC have authorized for you.
  • Although your instructor will receive an automatic email, get in touch with your instructor(s) at the beginning of the quarter and discuss the testing process and your accommodations with your instructor(s).

2. During the quarter, do the following:

  • Remind your instructor five (5) days prior to your exam that you plan to use your testing accommodations.
  • Ensure that your professor sends the exam and Proctoring Form to the SHAC Testing Services before you arrive to take your test.
  • Contact our office to make an appointment at least one week in advance.
MAKING AN APPOINTMENT

Appointments are required. At least one week in advance, call the SHAC Testing Services to schedule an appointment at (503) 725-5301. Additionally, check with our office before your appointment to ensure the exam has arrived.

TEST DAY

You must be on time for your test appointment and ready to take your exam. On your test day, bring the following:

  • A photo ID.
  • Permitted testing materials, such as a calculator or scantron.
RESCHEDULING

If, for any reason, you cannot take the test at the scheduled time (e.g. illness), you will need to:

  1. Notify your instructor as early as possible.
  2. Contact the SHAC Testing Services to cancel your appointment. You are responsible for rescheduling your appointment.
