Learning Disorder and ADHD Assessment

The mission of the LD/ADHD Testing at the Center for Student Health and Counseling (SHAC) is to reduce barriers to learning for at-risk PSU students. We offer high quality, low-cost assessments so that students may be eligible to access other campus resources, such as learning accommodations through the Disability Resource Center (DRC).

LD/ADHD ASSESSMENT CONSULTATION & REFERRAL

Step 1: Assessment Consultation Packet

Complete and gather all of the following to submit to the SHAC front desk:

Please submit the above completed materials to the SHAC front desk at University Center Building 200 and schedule an Assessment Consultation Appointment, which is typically available within 1-2 weeks.

Step 2: Assessment Consultation Appointment (2 hours)

You will meet with a psychologist to discuss your symptoms and education/medical history. Based on this consultation (and records, as available), the psychologist will provide you with recommendations for assessment and/or treatment.

   
LD/ADHD ASSESSMENT AT SHAC 

Step 1: Assessment Questionnaires

This and subsequent steps are only for those who go through the entire assessment process at SHAC and are not referred out to the community for testing. A set of questionnaires will be left at the SHAC Front Desk for you to pick up and return as soon as possible.

Step 2: Confirmation Appointment (30 minutes)

When contacted, schedule a 30-minute Confirmation Appointment to meet with the Assessment Administrator to:

  • Review policies, billing, and insurance information.  Be sure that you have contacted the Student Insurance Coordinator, Christi Ziegler, at (503) 725-2467 or Christi.Ziegler@pdx.edu to confirm coverage before the appointment.
  • Schedule or confirm testing appointments. Bring your planner.
  • Complete any questionnaires not previously turned in.

Step 3: Testing (11-13 hours)

You will be escorted to Testing services (UCB, Suite 310) to schedule proctored tests. It takes about 3 hours of testing that covers reading skills, study skills, and emotional functioning.

After the initial testing, you will meet with your assessment clinician (as previously scheduled at the Confirmation Appointment) 3-6 hours each week until testing is complete. The appointments include tests of various cognitive abilities and academic skills.

Missed appointments will result in $25 per hour no-show fee and may result in early billing for the assessment. It can also result in going back to the waitlist and postponement of assessment completion.

    Step 4: Preliminary Feedback & Fees (2-3 weeks after testing is complete)

    You will be contacted to schedule a preliminary feedback appointment once individual assessment is complete and assessment clinician's supervisor has approved the diagnosis.

    Fees and Insurance Reimbursement:

    • At this appointment $500 will be charged to your student account.
    • You will be given a receipt, if eligible, to send to your PSU student insurance for reimbursement. NOTE: you cannot receive a receipt until you attend a feedback appointment.

    Step 5: Final Feedback & Referral (4-6 weeks after testing is complete)

    You will be contacted to schedule a Final Feedback appointment once the report is complete and signed by the supervisor. You will be given a copy of the report. If appropriate, you will be referred to the Disability Resource Center (DRC) to initiate learning accommodations. Please note: An Authorization to Release Information form must be signed before your report can be sent to DRC or any other office or person.

    Depending upon assessment results, you may be referred to a variety of PSU and community resources.

    FEES You will not be charged to see a psychologist in SHAC. However, there is a $500 testing fee if you choose to test with SHAC, which will be billed to your student account at your preliminary feedback appointment as described in Step 4 above. You are responsible for contacting PSU Aetna Insurance to check for coverage and exclusions. (Students who take 5 or more credit hours are enrolled for Aetna insurance through PSU, and Aetna may pay for assessment within SHAC or with a private provider.)
