Number and Title

GO-7

= Remove Conditional Status
= Request for Change of Status

Students should file the GO-7 as soon as the specified requirements have been fulfilled.

No advisor signature required for GO-7

GO-10

= Reservation of Graduate Credit

for credits that have been reserved as graduate credits (although taken when student was an undergraduate)

GO-12

= Approved Graduate Degree Program (list of courses taken).

Adviser must submit the GO-12 during the first week of the quarter prior to the quarter when you will graduate.

  • For example, if you will graduate Spring quarter, file GO-12 in the first week of Winter quarter.

Students should not list MA-prerequisites LING 390 or LING 492 on the GO-12. Also, there is no need to list courses that are Ôextra' and do not count toward the degree. The Grad Office appreciates the bare minimum proof that students have satisfied the MATESOL degree requirements.

If it is unclear which degree requirement a course would satisfy (e.g., a topical course under LING 510), the adviser should write a short statement of clarification on the GO-12 form, in the space provided.

Save time and headaches! Advisor should write a memo with the GO-12 - If the advisor approves the substitution of one course for another, or if in any way a student's program of study looks unusual, the advisor should write a memo to accompany the student's GO-12. This will prevent problems when the Grad Office needs to determine how the student has fulfilled requirements.

GO-15

= Validation of Out-of-Date Graduate Credit for Master's Degrees

Courses that are more than 7 years old at the time of graduation, but no more than 10 years old at the time of graduation, can be used for the M.A. degree only after a validation exam.

After the oral or written exam, a GO-15 form must be filled out and submitted with the required documentation.

GO-16M

= Appointment of Final Oral Examination Committee (Masters Level)

GO-16M must be filed

  • by the student's Culminating Experience advisor (thesis, project, or exam)
  • during the first week of the quarter in which the student intends to graduate.

The advisor needs a copy of :

  • the student's Human Subjects approval (if applicable)
  • the date of the student's completion of the Foreign Language Proficiency exam from the Dept. of Foreign Languages and Literature.

IMPORTANT:

  • GO-16M cannot be processed until GO-7, GO-21 and GO-12 have been approved.

GO-17M

= Masters Recommendation for the Degree

This form is NOT handled by students -- and is only  handled by office staff and faculty.

GO-21

= Proposed for Master's Pre-admission and/or Transfer Credit

  •  
    • for graduate credits taken at PSU before the student was admitted as a graduate student
    • and for transferring credits from another university.

Submit GO-21 as soon as possible to get the credits approved.
