**Students requesting a service or assistance animal must complete both service or assistance animal AND disability request forms.

The student must register with the PSU Disability Resource Center (DRC), (503) 725-4150 (Voice); (503) 725-6504 (TTY or Relay). The DRC determines if a student is eligible for services. All information/documentation that is submitted by a student will be considered confidential and held in the DRC. Further documentation from a licensed or board certified medical or mental health provider must be submitted along with this request.
