Positive Youth Transitions (PYT)

In 2009, 40% of youth entering the Multnomah County Homeless Youth Continuum indicated foster care experience. The goal of the Positive Youth Transitions (PYT) Project is to change our collaborative systems so that no young person transitions out of foster care into homelessness. Ongoing support for systems collaboration has been funded through a gift to the PSU School of Social Work from the Joyce N. Furman Charitable Trust.

To benchmark our work, we will look at the 429 young people currently in foster care who are most at risk of homelessness - those with the designation of Another Planned Permanent Living Arrangement (APPLA). These youth are disproportionally African American and American Indian/Alaskan Native and we know that many have disabilities. We anticipate that the system changes made will benefit all youth.

In March 2012, 130 individuals from various organizations and agencies serving youth came together to begin the process of systems change. Currently, 13 action plans are moving forward. We will reconvene on September 13th, 2012, from 9:00 a.m. - 12:00 p.m. Please watch this site for more details.
