  • What does it say on the back of my card ?

This card is not transferable and remains the property of Portland State University until surrendered. This card is for purposes of identification and transaction of University business only and must be presented upon request by a properly identified University official. Report loss or theft to Campus Public Safety (503)725-4407. You are responsible for transactions made with this card until the theft or loss is properly reported. If found, mail to: PSU ID Card Office, P.O. Box 751, Portland, OR 97207. Do not punch fold or spindle, contains computer access chip.

 

  • You have been notified that ID Services has your photo ID card.

Due to security reasons, ID Services is unable to re-mail or forward cards to alternate address. You must come to the ID Services window located in Neuberger Hall, have your identity verified, to pick up your card.
